My minister spoke once of the need to confess sins in a specific way. While God searches our heart and knows things we don't know, it is beneficial to us to be detailed in our confession so healing and change can be complete. Forgiveness comes from God- even the desire to forgive ourselves and move on with our lives. This song is basically saying, "I believe Lord, help my unbelief" and asking Him for forgiveness and mercy.
